Thank you for choosing our office. Our goal is to provide high quality patient care in
an efficient and pleasant environment.
In doing so, we would like to provide you with some important information regarding our policies.

AFTERHOURS: In the event of a medical emergency you should dial “911” or go to the nearest emergency room. If you prefer to contact the physician on call, you may do so by calling the office number and selecting the appropriate option.

Our office handles a high volume of calls. Please use the voicemail system, in which you are offered different options and your call will be directed to the appropriate extension. Calls are returned within a timely manner.

OFFICE HOURS: Monday- Friday 8am - 12pm and 1pm - 5pm.

Please schedule your routine appointment 2-3 weeks in advance. Urgent Care appointment will be triaged and scheduled accordingly. When calling to schedule your appointment, please provide us with your name, approximate time and date in which you would like to be scheduled, reason for your appointment, insurance name and daytime phone. Should you need to cancel or reschedule your appointment please do so 24 hours in advance. We also encourage all patients to arrive 15 minutes prior to appointment time, providing any demographic or insurance changes.

Please contact your provider’s office at least 7-10 business days prior to your appointment with a specialist. Providing specialist name, date/time of appointment, office phone and fax, if possible reason for visit. Every effort to have your referral ready will be made, within 7-10 working day for non-urgent.

REFFERALS

When needing refills, please contact your pharmacy. WE HIGHLY RECOMMEND YOU TO ALLOW YOUR PHARMACY TO CONTACT US VIA FAX.

We will complete Disability and FMLA forms. The fee for all forms is $25.00. The fee is expected at the time forms are picked up. Forms can be drop off or mailed, please be sure to include a number where you can be reached once the forms are completed. Please have your portion of the forms completed prior to drop off and mail.

DISABILITY/FMLA FORMS

All medical records request are processed through ScanStat. Please fax medical records request to ( 713) 877-0970 and allow 7-10 days for processing. To check status of medical records please call ScanStat at (770) 569-2445.

Our office is compliant with HIPPA (HEALTH & HUMAN SERVICES HEALTH INSURANCE PORTABILITY AND ACCOUNTABILITY ACT). Should you require a copy or wish to view our Privacy Policy, please see the receptionist.

HIPPA

Payment is EXPECTED at the time of service, unless arrangements are made with our billing department prior to your appointment.
CASH, CHECKS, VISA, MASTERCARD, AND AMERICAN EXPRESS are accepted. Our office charges a $50.00 returned check fee.

PAYMENT

Note: We cannot release or discuss in any way information with anyone not designated on the HIPPA or medical record release form.

We will file your insurance claim as required by our contact with your managed care provider. You are responsible for any deductible, copay, co-insurance or non-covered charge, as stated in your insurance contract, at the time of service. If we are unable to verify your insurance benefits, you will be held responsible for services rendered. It is very important that you notify our office of any changes in your insurance coverage or demographic information. Failure to do so will result in services rendered being billed to you for payment. Should your insurance require additional information from the insured and such information is not received, you will be responsible for payment.

We will not file third party insurance claims (i.e. vehicular claims, or property insurance). You will be responsible for payment at the time of service.

We DO NOT see Worker’s Compensation injuries.
